​Focus

The primary focus is on rehabilitation plans and cross-sector communication concerning patients' rehabilitation needs and health literacy. Efforts are directed towards optimizing and implementing adjustments to these plans in close collaboration with local municipalities to ensure a seamless transition between sectors and that patients receive appropriate interventions within municipal rehabilitation centers.

Examples of Ongoing Work:

  • Quality Assurance: Continuous quality assurance of documentation by physiotherapists and occupational therapists in the rehabilitation plans.
  • Content Optimization: Collaborating with municipalities around Amager and Hvidovre Hospital to enhance the content of rehabilitation plans.
  • Assessment of Health Literacy: Assessing patients' health literacy within rehabilitation plans, including the development, and testing of a dialogue tool. Plans are in place to implement this dialogue tool across more municipalities and hospitals in the Capital Region.
​These initiatives aim to improve patient outcomes by ensuring effective communication and coordination between healthcare sectors, enhancing the quality and relevance of rehabilitation plans, and addressing patients' health literacy needs comprehensively.
