Focus

The focus is on patients hospitalized in departments such as geriatrics, cardiology, pulmonology, endocrinology, and infectious diseases. This also includes patients referred for outpatient rehabilitation in areas such as cardiac and pulmonary rehabilitation, as well as fall prevention.

The development and implementation of the following are key priorities:

Assessment and measurement of functional capacity: This includes initiating appropriate interventions to prevent functional decline during hospitalization.
Optimization and enhancement of physical activity: Special emphasis is placed on older adults, patients with pulmonary complications, and those who are not independently mobile.
Evidence-based rehabilitation interventions: These aim to improve patients' fitness, endurance, muscle strength, and balance within specialized rehabilitation settings.
Screening and treatment of vestibular and autonomic nervous system dysfunction: This is conducted in close collaboration with interdisciplinary partners in the fall prevention clinic.
Screening and treatment of pulmonary dysfunction and complications.

These initiatives aim to enhance patient outcomes by preventing functional decline, increasing physical activity, and providing tailored, evidence-based interventions.

Examples of Ongoing Work:

Dizziness and fall risk screening: All older patients are screened for dizziness and fall risk during hospitalization, followed by appropriate fall prevention assessments and interventions.
Increasing physical activity in the geriatric ward: Initiatives include testing electronic activity boards, physiotherapist-facilitated mobilization for breakfast, and participation in “The TALK-ACTIVE project".
Focus on Cultural Sensitivity: This approach to healthcare delivery emphasizes recognizing and respecting each patient's unique cultural background.
Frailty Screening: Identifying frail patients to prioritize their care during hospitalizatio
