Focus
The focus is on patients admitted to the Emergency Department and the Intensive Care Unit (ICU). Our efforts concentrate on the development and implementation of the following initiatives:
Functional Capacity Assessment: Conducting assessments in the Emergency Department and initiating appropriate interventions to prevent functional decline during hospitalization or after discharge, including the provision of aids and referrals to municipal rehabilitation and outpatient fall prevention programs.
Critical Illness Rehabilitation: Collaborating closely with interdisciplinary partners to assess and rehabilitate critically ill patients in the ICU.
Frailty Screening: Identifying frail patients and implementing relevant interventions.
Pulmonary Complications Management: Screening and treating pulmonary complications and dysfunction.
Examples of Ongoing Work:
Fall Risk Screening: Assessing dizziness and fall risk in the Emergency Department to implement appropriate fall prevention measures and interventions.
Frailty Identification: Screening for frailty to prioritize care for vulnerable patients during hospitalization.
Physical Function Assessment: Implementing “The Chelsea Critical Care Physical Assessment Tool (CPAx-D)" for evaluating physical function in critically ill patients in the ICU.
